Job Description

Mental Health Kokua (MHK) is hiring a Case Manager in Hilo (Hawai ʻi Island) to support adults with mental health challenges through community-based services. This full-time, community-based role focuses on helping individuals build stability, strengthen independence, and access the services and resources needed to support their recovery goals. As a Case Manager, you will coordinate care, advocate for client needs, and connect individuals to behavioral health, medical, housing, and community-based supports. This role is well-suited for compassionate professionals in Hilo and surrounding East Hawaiʻi communities who enjoy relationship-based work and making a meaningful impact through recovery-oriented services. What You ll Do (Responsibilities)
Care Coordination:
Assess client needs, develop service plans, and coordinate behavioral health, medical, housing, and community resources
Client Advocacy:
Support individuals in accessing benefits, services, and community-based supports that promote stability and independence
Recovery-Oriented Support:
Promote recovery using person-centered, strength-based, and trauma-informed practices
Hospital & Transition Support:
Provide follow-up services and support successful transitions following hospitalization or other major life changes
Community Collaboration:
Partner with providers, families, landlords, and community organizations to support client goals
Crisis Response:
Respond appropriately to client crises and coordinate support services as needed
Documentation & Compliance:
Complete timely assessments, service plans, progress notes, and required documentation
Team Collaboration:
Participate in supervision, case reviews, and interdisciplinary team meetings to ensure quality services
Case Manager Qualifications Preferred:
Bachelor s degree in psychology, sociology, social work, human services, or a related behavioral health field
Preferred:
One and a half (1.5) years or more of case management, human services, or behavioral health experience; experience working with co-occurring challenges is a plus
Skills:
Strong communication, organization, documentation, advocacy, and relationship-building skills
Certifications:
TB clearance, CPR, and First Aid (or willingness to obtain) Fit for the
