We need mission-driven team players with Case Management and/or Outreach experience to provide services to new clients. Share in IHS's legacy of service to those most in need!
Responsibilities:
Create and offer tailored solutions for those in crisis, and nurture homeless people toward greater self-direction and responsibility. Duties include supportive counseling/case management, outreach, and provision of safe, decent and affordable housing opportunities, skills teaching, family support, service coordination/advocacy, linkage to community-based services and crisis support for IHS's homeless population. Work collaboratively with the treatment team to implement enrollment to HMIS/VI-SPDAT, assessment at admission/at discharge when appropriate.
Requirements:
Working knowledge of the case management process, assessment, planning and coordination of services. Knowledge of community resources, levels of care, as well as the standards for care within the community resources. The ability to assess physical and psychological factors impacting the case and how to coordinate service delivery. Possess personal qualities of integrity, patience and commitment to mission. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.
Required Education & Experience:
High School diploma or GED equivalent One year of direct case management or supportive services providing linkage to community resources 1 year of experience working with homeless or other disenfranchised individuals with mental health and/or substance abuse issues Valid Drivers License with clean traffic abstract
Strongly Preferred Education & Experience:
Bachelor's degree in Human Services, Social Work, Psychology or related field with 12 semester credit hours of Sociology, Psychology, Human Services etc. Two years of direct case management or supportive services 1-2 years of experience working with Homeless or other disenfranchised individuals with mental health and/or substance abuse issues CPI and CPR certified IHS is an Equal Opportunity Employer, maintains a Drug and Alcohol-free Workplace, and an employer of national service.
