General Summary:
The Intensive Case Manager reports to the Program Manager for the Pathway Home program funded through the Department of Labor, Pathway Home 4 grant. The Intensive Case Manager is a critical component of the successful launching of the program and involved in all aspects of implementation and program execution. The program is designed to provide reentry case planning, comprehensive case management, occupational training, and workforce services to 100 incarcerated persons residing at the Seventh Judicial District, Department of Corrections - Work Release Center and/or Residential Correctional Facility. In addition to core services, supportive services are provided through community partnerships to assist enrolled participants. The Case Manager will be a key member of the Pathway Home team, providing assessments, development of individual employment plans, participant collaboration and engagement, coordination of supportive services, as well as contributing to the overall design, start-up, and implementation of services. Exceeding Expectations, Communication and Collaboration, Integrity and Evidence Based Innovation.Essential Duties and Responsibilities:
Stakeholder Interface and Service Conduct intake/Safer registration, creating file for documentation and ensuring all relevant releases and paperwork are signed and placed into the client file. Provide intensive case management - ensuring the appropriate number of engagements according to level of risk/need. Coordinate and provide warm referral to supportive services (housing, behavioral health, legal, etc.) if assessed as beneficial. Develop Individual Employment Plan that incorporates an education plan that directly addresses needs of participants, updates on a regular basis, or after the completion of goals, or other life changing events. Link participants with instructional facilitator to increase basic skills and/or pursue high school equivalency diploma. Connect participants with Employment Development Specialist (EDS) to engage in Safer Skills Training (SST - job readiness training) in a group/individual format. Work with the EDS to assist participants in creating resumes, applying for jobs on-line, employment placement and retention tracking. Identify alternative funds for training including Pell grants, regional workforce funds and other revenue streams to cover the costs of participant training, utilizing budgeted training dollars as a last resort. Work with community college and other community partners to provide case management services for all participants according to Individual Employment Plan. Perform other duties as assigned. Stakeholder Outreach and Recruitment Meet client recruitment goals through referral and screening for eligibility, utilizing risk/needs assessment tool. Attend partner meetings as appropriate. (Community Colleges, supportive services, etc.) Attend internal meetings and work in collaboration with QC team to create awareness, promote Safer's mission in the community and advocate for the population Safer serves. Participate in agency wide collaborative efforts as assigned. Maintain high standards of professional and ethical conduct. Establish and maintain a good working relationship with community stakeholders/referral sources. Tracking, Document, and Reporting In collaboration with participant, develop Individual Employment Plan which is reflective of identified needs and goals. Document referrals for supportive services Complete assessments using standardized assessments tools. Based on assessment results, provide appropriate referral to services designed to assist with identified needs. (e.g., instructional facilitation, financial literacy, job readiness, etc.) Enter data accurately and timely into Salesforce system, tracking client progress through case notes or activities within 48 hours of engagement. Maintain audit-ready electronic and hard copy files at all times.Required Knowledge, Skills and Abilities:
Knowledge of the needs of people with arrest and conviction records, especially reentry populations and initiatives. Knowledge of human behavior and performance; individual differences in ability, personality, and interests, learning and motivation. Knowledge of group behavior and dynamics and societal trends and influences Demonstrated organizational and problem-solving skills Excellent verbal and written skills; able to effectively present information in individual or group settings. Aptitude for client engagement and intensive case management. Self-reliant, resourceful, and outcome-driven with the capability to function in a fast-paced, start-up environment. Hands-on experience and self-sufficiency with Microsoft productivity tools, including Outlook (for email), Office (Word, PowerPoint, Excel), and online collaboration tools (video conferencing, Zoom/Teams, messaging).Education and Experience:
Bachelor's degree in related discipline or equivalent work experience preferred. Backgrounds similar to the population served are especially desirable. Prior experience in start-up and/or intensive case management work environment.Licenses and or Certifications:
