Grant and Per Diem Service Coordinator Report To:
Homeless Services Supervisor Description:
The Service Coordinator is responsible for providing support and case management to participants in Humility Homes & Services Grant and Per Diem Program. The primary focus is assisting clients who are living in transitional housing setting in HHSI's emergency shelter obtain permanent housing by connecting them to eligible mainstream resources and other supports. This position works collaboratively and alongside the Veteran's Administration in ensuring the veteran is receiving supports to transition from homelessness to housing.
Qualifications:
Education Required:
Bachelor's Degree, with major in Social Services, or related fields, and preferred minimum of 2 years' experience in social services. Four years of verifiable case management in Social Services or Housing Navigation experience, may be substituted for the educational requirement. Must have valid US Driver's License.
Experience:
Individual must possess demonstrated experience with crisis intervention, Trauma-Informed Care, and social work practice with chronically homeless individuals and/or individuals with disabilities (mental illness, substance abuse/addiction, physical disability, developmental disability, chronic health problems, etc.) Individual must also be capable of preserving the confidentiality of employee and participant records. Preferred experience working with people or families who have experienced homelessness and have disabilities of long duration including, mental health, substance abuse, mental or physical impairment Individual must have a results-oriented background and the ability to think strategically, anticipate future trends in homeless services and ability to implement them. Must demonstrate an exceptional capacity for multi-tasking. Individual enjoys working hard and looks for challenges; able to act and react as necessary. Humility Homes and Services, Inc. Responsibilities:

•. Commitment to Ethical and Effective Services
• Know and implement HHSI's mission, policies, and procedures; and abide by all city, state, and federal laws, and regulations. Maintain required training to obtain and keep HMIS license including state required Coordinated Entry training. Maintain training in suicide risk assessment, mandatory child and dependent adult abuse and neglect reporting, crisis intervention, civil rights, housing discrimination, harm reduction, trauma informed care, housing first practices, boundaries, bloodborne pathogens, fire safety, cultural competency, workplace safety, active shooter, CPR/First. Aid, Verbal Judo, Trauma Informed Care, domestic violence training and Housing First. Assist the Program Supervisor by regularly evaluating housing programs for community relevancy, effectiveness, and outcomes. Collaborate with other VA Service providers to ensure proper placement of Veterans into programing. Maintains current knowledge of evidence-based practices as they relate specifically to serving veterans, individuals and families of whom are experiencing homelessness or housing instability. Regularly evaluate the needs of participants on caseload and provide them with ongoing support. Commitment to ensuring HHSI has an equitable and diverse working environment. Case Management:
Meets with program participants and creates an Individualized Housing Plan with them. Frequency and location of meetings determined by program requirements and individual need. Completes a housing need, and mainstream resources assessment with participants. Completes any other mandated assessments per specific grant agreement or program policy and procedures. Completes Assessments for Coordinated Entry with any participant who is literally homeless. Transports program participants to appointments, apartment showings and any other necessary locations based on their ISP. Connects participants to mainstream benefits and assists them in applying when applicable. Liaisons with landlords to either help a participant obtain housing or address any concerns once the participant is housed. Provides life skills training like laundry, navigating the bus system etc. for program participants. Provides crisis intervention and advocacy for participants, as needed. 3.
Documentation:
Completes daily case notes, service transactions, and contacts then uploads them to ServicePoint. Maintains current income verification, eligibility and services provided as required for specific grant agreement. Maintains releases of information for all participants and external providers when applicable. Completes fund requests for participant needs when applicable. Refers participants to other programs at HHSI for which they are eligible, along with supportive documentation. 4.
Other Duties:
Collaborates on case plans for high need participants with the Supervisor. Case Conferencing weekly with Supervisor and other VA Service Providers. Maintains knowledge of Evidenced Based Practices for specific program. Maintains strong knowledge of community resources, especially as it relates to housing. Regularly promotes the agencies' mission and advocates for social justice. Cross trained to cover the 1st shift Emergency Shelter Monitor position, Coordinated Entry, and Intake and Diversion Specialist position. Performs other duties, as needed
Schedule:
Full-Time. Must be able to work 36 hours per week. General hours are Monday through Friday between 7am and 4pm; however, must be flexible for earlier days or later nights as needed. This position will be housed within the Emergency Shelter, must be flexible in knowing that some nighttime hours may be required to connect with participants .

Position is also regularly required to use janitorial supplies, such as disinfectants, laundry detergent, air fresheners, etc. Shelter, office, and scattered-site housing may occasionally house service and emotional support animals.
