This role focuses on helping clients access services, build skills, and work toward recovery and independence.
You will:
Help clients identify strengths, needs, and goals Develop and implement individualized service plans Provide skill‑building, education, and support in the community Coordinate care with providers, families, and community resources Maintain accurate, timely documentation Complete 90‑day and annual reviews Respond to client needs promptly and professionally Participate in weekly supervision and quality assurance Join the 24‑hour on‑call rotation after the initial training period Complete 20 hours of continuing education annually Qualifications Bachelor's degree in a human services field (psychology, sociology, social work, etc.) Strong communication, organization, and problem‑solving skills Ability to work independently and as part of a team Commitment to ethical practice and professional growth Willingness to consult with supervisors and follow Medicaid/NASW guidelines Compensation & Structure Compensation is based on billable hours Most job duties are billable Supervision and QA participation are required but not billable Supportive leadership and structured training provided Why Join Our Team?
