Full-Time Transitional Housing Case Manager Location:
Moscow, Idaho Organization:
Sojourners Alliance Schedule:
Full-Time, Monday-Friday, 8:00 a.m.-4:00 p.m. Make a Difference Every Day Sojourners Alliance is seeking a compassionate, organized, and motivated Transitional Housing Case Manager to help individuals and families experiencing homelessness build stable, independent lives. What You'll Do As a Transitional Housing Case Manager, you will: Provide individualized case management to residents in our Transitional Housing program. Help participants develop and achieve housing stability goals. Connect participants with employment, healthcare, behavioral health, recovery services, benefits, and other community resources. Assist participants in locating and securing permanent housing. Monitor participant progress and provide ongoing support and accountability. Maintain accurate case notes, HMIS documentation, and program records. Collaborate with landlords, service providers, and community partners. Participate in team meetings and contribute to a positive, professional work environment. What We're Looking For Successful candidates will demonstrate: Strong interpersonal and communication skills. Compassion balanced with healthy professional boundaries. Excellent organizational and time management abilities. The ability to manage multiple priorities in a fast-paced environment. Professional documentation and computer skills. A commitment to treating every participant with dignity and respect. The ability to work independently while collaborating effectively with a team. Experience in social services, case management, behavioral health, housing, human services, or a related field is preferred, but individuals with relevant education or comparable experience are encouraged to apply. Qualifications:
Previous Case Management experience preferred BA in Social Work or related field, Masters preferred.
