Job Description

The Service Coordinator functions as a case manager providing a full range of strengths-based services and performs general needs assessments of residents living in public housing, facilitates community linkage, and provides resident instruction and training. The (SC) collaborates with public and private resources, service providers and agencies to deliver services and empowerment activities to meet residents' needs, including transportation, meals on wheels, home health aides, homemakers, financial assistance, counseling, preventative health screenings and other supports to ensure program participants receive the necessary services allowing for the removal of barriers for improved quality of life, independence and stability while residing in public housing. The Service Coordinator will create, coordinate and facilitate activities appropriate for residents, and will work to support and be a liaison to the resident councils.
Position Qualifications/Requirements:
Education:
Bachelor's degree in human services required with 2 years of experience in human services case management; MSW preferred. Applicants must have working knowledge of social service providers in the area and be able to create community linkages as well as the ability to communicate effectively with diverse populations. Experience in mental health is preferred. Individuals must have a valid driver's license and be insurable by the Authority's insurance carrier. Individuals are expected to utilize their own car for daily work related travel with mileage reimbursement for all miles incurred during service provision. The Service Coordinator position is non-exempt position with an expected 40 hour work week Monday - Friday, 8am - 4:30 pm with flexibility to engage with program participants as they are reasonably available. Applicants must be able to pass a drug test for employment with the Authority.
Salary/Benefits:
