Overview Join our dynamic healthcare team as a dedicated Case Manager , where you will play a vital role in coordinating client services and ensuring seamless transitions across various settings. This position offers an exciting opportunity to utilize your clinical expertise, social services knowledge, and exceptional communication skills to improve client outcomes. As a Case Manager, you will collaborate with multidisciplinary teams, manage complex cases, and advocate for clients from diverse backgrounds, all while adhering to regulatory standards and best practices . Duties Conduct comprehensive assessments of clients' medical, social, and emotional needs to develop personalized care plans. Coordinate discharge planning and facilitate smooth transitions from hospital to home or other care facilities. Collaborate with healthcare providers, social workers, insurance companies, and community resources to optimize client care and resource utilization. Perform utilization review and management to ensure appropriate level of care while maintaining compliance with policies . Utilize Electronic record keeping to document clients' interactions, progress notes, and care plans. Qualifications BS/BA in human services or related field or a RN or LPN. Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds. Pay:
$18.00 - $20.00 per hour
