Job Description
The QIDP/Case Manager is responsible for coordinating and managing services for individuals with developmental disabilities. This role involves developing and implementing individualized care plans, ensuring compliance with state and federal regulations, and advocating for clients' needs. The ideal candidate will have strong communication and organizational skills, along with the ability to work collaboratively with clients, families, and other service providers. Description We are seeking a highly organized and dedicated individual to join our team as a QIDP/Case Manager. As a QIDP/Case Manager, you will be responsible for coordinating and managing services for individuals with developmental disabilities. Your primary role will involve developing and implementing individualized care plans, ensuring compliance with state and federal regulations, and advocating for clients' needs.
Responsibilities:
Develop and implement comprehensive care plans for individuals with developmental disabilities, taking into consideration their unique needs and goals. Coordinate and monitor all aspects of client care, including coordinating medical appointments, therapy sessions, and other necessary services. Conduct regular assessments to evaluate clients' progress and adjust care plans accordingly. Ensure compliance with all state and federal regulations, including documentation and reporting requirements. Act as an advocate for clients, connecting them with appropriate resources and advocating for their rights and needs. Ensure compliance with all Medicaid waiver requirements as a QIDP. Collaborate with clients, families, and other service providers to ensure the delivery of high-quality and person-centered care. Provide ongoing training and support to clients and their families to promote independence and self-advocacy. Maintain accurate and up-to-date client records and documentation. Stay current with industry developments and best practices in the field of developmental disabilities.Qualifications:
Bachelor's degree in social work, psychology, or a related field. Prior experience working with individuals with developmental disabilities is highly preferred. Knowledge of state and federal regulations related to services for individuals with developmental disabilities. Strong communication and interpersonal skills, with the ability to work effectively with diverse populations. Excellent organizational skills and attention to detail. Ability to work independently and make sound judgments in complex situations. Proficient in the use of computer systems and software applications for documentation and record-keeping.
