Job Description
An ACT Case Facilitator I
- Adult operates under supervision of the Community Outreach Director or designated QBHP/LP.
Shift:
First shift, full time- 8a to 5pm Monday thru Friday.
Education:
High school diploma or equivalent.Experience:
Must meet OBHP requirements = Two years experience in mental health or community services preferred.License:
