Job Description

An ACT Case Facilitator I
  • Adult operates under supervision of the Community Outreach Director or designated QBHP/LP.
The Case Facilitator I will be a member of the ACT multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to adult consumers, client reassessment and completion of ANSA, recovery plans and reviews, referral and linkage to services, and client terminations.
Shift:
First shift, full time
  • 8a to 5pm Monday thru Friday.
Education:
High school diploma or equivalent.
Experience:
Must meet OBHP requirements = Two years experience in mental health or community services preferred.
License:
Must hold a valid state issued driver's license. Salary based on experience. Excellent benefits EOE ACT Case Facilitator I
