Full Job Description Educational Requirements:
Bachelor degree in the field of social work, or another related field of study.
Other Requirements:
Must possess a valid driver license Have dependable transportation Maintain car insurance
Position :
DCFC is looking for an individual who is able to complete case management within our foster care program. Responsibilities for
Foster Care Case Management:
A qualified case manager will have a Bachelor Degree, preferably in Social Work or a related field. Case Managers will work with foster families, foster children, the Department of Child Services and other service providers to obtain the best possible results for children in care. Case Managers will attend CFTM's, court hearings, IEP meetings, Provider Meetings and will at times transport the children on their caseload to these meetings,as well as to medical, dental and optical appointments. Case Managers are required to complete monthly reports, as well as, treatment plans and will be required to be on call for short periods of time.
