The Case Manager will provide community-based case management, skills training & development, and other treatment services to an adult outpatient population.

WHAT IS THIS POSITION RESPONSIBLE FOR?
Conduct case management, skills training & development, and other treatment services according to Southwestern policies. Provide case management services to adults assigned to Outpatient Services including, but not limited to communicating with all parties involved in the client's treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process. Provide emergency counseling and crisis intervention when necessary. Maintain appropriate records of services to reflect patient progress and document treatment plans and goals. Communicate professionally, make referrals, and coordinate services with community agencies. Must meet efficiency standards which include chart management and patient contact standards
WHAT'S REQUIRED FOR THIS POSITION?
Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field. Knowledge of available community resources is preferred. Previous work with the SMI population is highly preferred. Demonstrated proficiency with interpersonal and writing skills. Excellent communication, time management, and organizational skills. Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance. Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.
