Full Job Description Overview:
We are a growing emergency shelter serving children in the Fort Wayne area. Our mission is to provide a safe, supportive environment while helping youth stabilize and transition to long-term solutions.
Position Overview:
We are seeking a compassionate and reliable case manager to provide case management services, crisis support, advocacy, and coordination of care for youth residing in our facility.
Key Responsibilities:
Provide crisis intervention and emotional support to youth during placement. Assist youth with life skills development, problem-solving, conflict resolution, and goal setting. Monitor youth progress and document behaviors, incidents, and achievements. Assist with discharge planning and transition services. Ensure all documentation is completed accurately and within required timelines.
