Aftercare Case Manager

Description Average hourly rate is $19.23 (including wage, incentives, bonuses, overtime, shift differential, etc.) Do you love helping children and families thrive? TFI Family Services is hiring a full-time Aftercare Case Manager to join our team! What You'll Need A bachelor's degree in human services . A valid driver's license . A willingness to learn—we'll provide all the training you need! A Day in the Life As an Aftercare Case Manager, you'll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
You'll:
Focus on risk management and help families maintain stability during the aftercare period. Provide crisis intervention and work with public and private agencies offering supportive services. Maintain accurate documentation and reports. Coordinate community and educational services for families. Meet with families in-person to support reunification and resolve any concerns.
