Case Manager
Job Description
Job Overview DPOK is a growing nonprofit organization seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will support people with intellectual and developmental disabilities (I/DD) by assisting them to access services, coordinating individualized supports, maintaining accurate records, and advocating for their needs. The ideal candidate is dependable, detail-oriented, and committed to promoting independence, choice, and a high quality of life for people with I/DD. This is a full-time position with a flexible schedule that includes being off work at 12:30 p.m. on Fridays. On-call is possible. The position includes a competitive benefits package. A minimum of six months of full-time experience in the field of human services; and either a bachelor's degree or additional full-time experience in the field of developmental disabilities services, which may be substituted for the degree at the rate of six months of full-time experience for each missing semester of college. Duties Coordinate services and supports based on each person's goals, preferences, needs, and strengths. Advocate for people and help them access community resources, benefits, healthcare, and other services. Develop, review, and update person-centered plans and related documentation. Maintain accurate and timely case notes, records, intake assessments, and service documentation. Communicate regularly with people, families, service providers, professionals, and other members of the support team. Monitor services to ensure they are meeting established goals and follow up on concerns or changes in needs. Conduct visits and meetings as required to assess well-being, safety, and progress. Protect confidential information and comply with applicable privacy, documentation, and reporting requirements. Respond to concerns professionally and assist with resolving barriers to success. Skills Proven experience in case management, social services, human services, education, or a related field. Knowledge of intellectual and developmental disability services and community resources. Strong communication, advocacy, problem-solving, and communication skills. Ability to manage multiple responsibilities, meet deadlines, and maintain accurate records. Proficiency with Microsoft Office and the ability to learn new software applications quickly. Ability to work independently while collaborating effectively with people with I/DD, families, and service teams. Valid driver's license and ability to travel to meet with people where they are at.
