This role focuses on supporting clients in achieving stability and self-sufficiency through individualized service planning, coordination of care, and connection to community resources. Work hours are 7 am to 3 pm, Monday-Friday Duties and Responsibilities
- Assists clients in development and implementation of a care plan that is tailored to the client's specific needs and is designed to improve basic functioning in daily life
- Acts as a liaison, client advocate, and coordinator among those involved with the client's care, including medical, community placement, and mental health providers
- Monitors entire work environment, ensuring the facility is kept as clean and orderly as possible
- Arranges for client transportation to supportive services
- Works in collaboration with other social service providers and non-profit agencies
- Provide accurate reports used by the Department of Housing and Urban Development (HUD) through monthly rental logs, quarterly HUD reports, and annual APR reports
- Maintain bi-weekly case notes and complete other documentation as required Participates in the on-call rotation as required
- Performs all other duties assigned by a supervisor Qualifications, Expectations, and Competencies Qualifications Associate degree in behavior science field; or a combination of education and equivalent work experience in directly related field 2+ years of experience with Microsoft Office
Valid Kansas Driver's License Preferred:
Bachelor degree in behavior science field; or a combination of education and equivalent work experience in directly related field

- Strong work ethic Working Conditions and Physical Requirements The position is regularly required to make use of close vision to read correspondence and prepare reports and other appropriate communications.
While performing the duties of this job, the employee is frequently required to sit, speak with employees and clients, and use a computer for extended periods throughout the day. The employee is occasionally required to stand and walk. S/he must hold a valid driver's license without restrictions. May be required to acquire and maintain a Food Handler's certification.
This position will also be required to occasionally assist in the physical preparation of events, apartments, grounds or similar activities. Must be able to lift 25lbs with or without assistance. Work hours for this position will vary based on assigned location. Some activities require evening and weekend attendance outside of the normal working hours. Overtime may also be required.
