Job Description

Twin Rivers Developmental Supports, Inc., is currently accepting applications for a Case Manager. The Case Manager is responsible for counseling and coordinating services for individuals on an assigned case load. This is a full-time position working 8:00 am.to 4:30 p.m. Monday through Friday.
Job Duties:
Obtain and maintain a license with Health Care Policy to provide Case Management. Write and implement plans and programs. Serves as contact for parents/guardians and other professionals. Abide by the Health Care Policy Rules of Conduct for Case Managers. Assist with securing needed support and ancillary services. May be required to assist as back up, fill in and behavior management. Transporting individuals to and from appointments as needed. General office duties. Other duties as assigned.
Qualifications :
Bachelor's degree (full-time experience in the field of developmental disabilities may be substituted for a degree at the rate of six months full time experience for each missing semester). One year of experience in the field of developmental disabilities.
