Job Description

Salary - $35,000 - $39,375
Position Summary:
The Targeted Case Manager is responsible for providing case management services to assigned consumers to support their overall well-being and progress toward recovery goals. This includes coordinating resources, maintaining documentation, and ensuring adherence to all Pathways policies and procedures.
Educational Requirements:
Bachelor's Degree in Human Services or a related field Minimum of one (1) year of experience in Human Services following graduation Equivalent experience in a related field may be considered
Essential Duties and Responsibilities:
Provide case management services to assigned consumers based on individualized service plans Coordinate and link clients to appropriate community resources and services Monitor client needs and progress, updating service plans as appropriate Maintain timely and accurate documentation using electronic health record systems Participate in staff meetings, training sessions, and interdisciplinary team discussions Ensure adherence to professional standards, ethics, and confidentiality requirements Perform other duties as assigned
Skills and Knowledge Required:
Experience in a behavioral health setting is a plus Competency in using computers and electronic devices , including electronic health records Strong organizational, communication, and time management skills Ability to work independently and collaboratively with diverse populations Knowledge of local community resources is beneficial
Preferred Qualifications:
Prior case management or social services experience Familiarity with behavioral health systems, services, and terminology
