The ideal candidate will play a crucial role in supporting individuals and families in navigating complex social services systems. This position requires strong interpersonal skills, a commitment to advocacy, and the ability to manage crises effectively. The Case Manager will work closely with justice impacted, foster or homeless and runaway youth and young adults by assessing their needs, developing personalized care plans, and connect them with appropriate resources. This is an in-person/on-site position that requires flexibility in availability. Responsibilities Conduct comprehensive assessments of clients' needs and circumstances. Develop, implement, and monitor individualized service plans. Provide crisis management and intervention services as needed. Facilitate access to community resources such as healthcare, housing, and childcare. Offer support in areas such as addiction counseling and child welfare. Maintain accurate documentation of client interactions and progress. Collaborate with multidisciplinary teams to ensure holistic care for clients. Supervise and mentor junior staff or interns when applicable. Engage in motivational interviewing techniques to empower clients toward positive change. Experience A background in social work or a related field is preferred. Experience in crisis management and intervention is essential. Familiarity with Medicare processes and child welfare systems is a plus. Previous experience in supervising or mentoring staff is desirable. Strong communication skills, both verbal and written, are required. Ability to work effectively with diverse populations and demonstrate cultural competence. Knowledge of youth development, trauma, and addiction counseling will be beneficial.
