Healthy Start Case Manager

Family Road of Greater Baton Rouge is seeking a Case Manager to provide home visitation/care coordination services to pregnant and parenting mothers and their families. The case manager provides comprehensive case management services, health education, psychosocial assessment, community resource/referral and program outreach/recruitment. Case managers must have exceptional communication/ interpersonal skills, must be comfortable with providing public program presentations, have the ability to interact effectively with multi-disciplinary team members and program participants, must be detailed oriented, have excellent organizational/time management skills, comprehensive knowledge of community resources, experience in mental health/crisis counseling, and must have knowledge of the Healthy Start program design, implementation, management, monitoring and required performance and reporting. Candidate must be proficient in data collection/data entry, must have exceptional computer skills with experience in using Microsoft and other computer applications, and knowledge of social media communication. Candidate must have flexibility to travel for occasional trainings and professional development. Preferred experience in working with maternal and child health populations. Position requires a Master's degree in Social Work with 2-3 years of case management experience in a social service setting and a current LA License (LMSW) or a Bachelor's degree in Social Work (RSW) and 3-5 years of case management experience in a social service setting will be considered. A Master's degree in another discipline with professional level social service experience can also be substituted. Salary is based on education level and experience.
