Job Description

We are seeking a dedicated and dependable Family Support Coordinator/Case Manager to support children and families participating in the Early Steps program. The ideal candidate will hold a bachelor's degree from an accredited institution in a human services-related field, such as Psychology, Sociology, Social Services, Philosophy, Criminal Justice, Family and Consumer Sciences, Rehabilitation Services, Child Development, Counseling, or Social Work. Candidates must demonstrate professionalism, maturity, and reliability. Key Responsibilities Maintain monthly communication with recipients' parents or guardians to monitor progress and address concerns. Facilitate quarterly face-to-face team meetings with families and service providers. Collaborate with service providers to ensure children ages birth to three receive appropriate services and supports to achieve developmental milestones. Develop, review, and update Individual Family Service Plans (IFSPs), including initial, six-month, and annual assessments. Maintain accurate, organized, and timely documentation in recipient records. Prioritize tasks effectively while managing multiple responsibilities and meeting deadlines. Participate in all required trainings and professional development activities. Utilize reliable transportation to fulfill job-related travel requirements. Demonstrate strong organizational, written, and verbal communication skills. Maintain proficiency in Microsoft Office applications, including Word and Excel, and other computer-based systems as needed. Qualifications Bachelor's degree in a human services-related field from an accredited institution. Strong interpersonal and case management skills. Ability to work independently and as part of a multidisciplinary team. Reliable transportation and willingness to travel as required. Proficiency with Microsoft Office and general computer applications. Ability to manage competing priorities while maintaining attention to detail and quality of service.
