We're looking for a dedicated and organized Case Manager (QIDP) to oversee client services, support residential programs, and ensure each individual receives high-quality, person-centered care. About the Role As a Case Manager at Magnolia, you will serve as a key member of the interdisciplinary team, managing a caseload of residential clients and overseeing the development and implementation of Individualized Service Plans (ISPs). You'll coordinate care, support group home operations, supervise Direct Support Professionals (DSPs), and work closely with families and internal teams to ensure the best outcomes for those we serve. Key Responsibilities Develop, implement, and monitor Individualized Service Plans (ISPs) to promote client independence and quality of life Track client progress and complete quarterly reviews and reports Coordinate services and supports in alignment with ISP goals and physician recommendations Advocate for clients and ensure their needs are met across all areas of care Communicate health concerns and updates with the wellness team and interdisciplinary staff Attend and support medical appointments as needed Review and investigate incident reports in accordance with policy Manage client funds, maintain accurate records, and ensure compliance with financial guidelines Provide training in budgeting, daily living skills, and behavior support strategies Conduct routine group home inspections to ensure safety, cleanliness, and compliance Submit and track maintenance requests and ensure proper documentation is maintained Provide functional supervision, training, and support to DSPs Ensure accurate and timely documentation in required systems Serve as the primary point of contact for families, guardians, and external stakeholders Coordinate home visits, transfers, and discharges Promote a positive, respectful, and person-centered environment Requirements Bachelor's degree in a field that meets regulatory requirements At least one year of experience working with individuals with intellectual or developmental disabilities Strong communication, organizational, and time management skills Ability to prioritize tasks and make sound decisions Ability to remain calm and professional in challenging situations Basic technology skills for documentation and communication Willingness to learn Medicaid, ICF/IID, and applicable regulatory standards Must be 21 years of age or older Valid Louisiana driver's license and current automotive insurance required Acceptable Motor Vehicle Record CPR certification required Must pass a background check, pre-employment drug screening, and physical Availability for occasional evenings, weekends, and on-call support as needed If you're a compassionate and organized professional ready to take the lead in supporting adults with developmental disabilities, apply today and join Magnolia in making a lasting impact!
