YOUR ROLE AS A INDIVIDUAL SUPPORTCOMMUNITY HABILITATION WORKER
As an Individual SupportCommunity Habilitation Worker at Guardian Angel Senior Services, you will play a pivotal role in empowering individuals to actively participate in decisions regarding their education, health, and care provision. By working towards the goals outlined in each personalized care plan, your responsibilities will include: Assisting with budgeting Arranging transportation, Managing medical appointments, and obtaining essential core documents. Collaboration with the interdisciplinary team and fostering partnerships between individuals and the community are critical to achieving better outcomes. You will provide resources that help clients maintain safety and independence while undertaking home visits and documenting all activities comprehensively. Additionally, you'll assist in training and supervising caregivers and PCAs, ensuring compliance with state and federal regulations, and maintaining utmost confidentiality. By effectively managing your time and adhering to agency policies, you will contribute to a supportive environment that champions both individual growth and community integration.
WHAT WE'RE LOOKING FOR IN A INDIVIDUAL SUPPORTCOMMUNITY HABILITATION WORKER
To succeed as an Individual SupportCommunity Habilitation Worker at Guardian Angel Senior Services, you will need: a minimum of a BA in a Human Services-related field, coupled with strong IT skills to manage websites and utilize various forms of social and media communications effectively. Essential communication and listening skills are critical, enabling you to engage with clients while producing accurate statistics and written reports as required. An ability to research and identify resources for additional information, advice, or guidance is also vital. You should be flexible regarding working hours, as some evenings and weekends may be necessary. A reliable vehicle for independent travel throughout the borough is preferred, and knowledge of the community support system in the Merrimack Valley area will be an asset. Furthermore, you must possess a commitment to the rights of seniors and disabled individuals, demonstrating a non-judgmental and impartial approach while remaining sensitive to their needs amidst emergencies. Prior home care experience can further enhance your ability to deliver exceptional support. Knowledge and skills required for the position are: Minimum of a BA in a Human Service-related field preferred. Have an ability to research and identify other sources of information advice guidance and support Have good IT skills including managing websites and knowledge of all forms of social/media communications Have good listening and communication skill Be able to produce statistics and written reports as required Be able to travel around the borough independently preferably by car Be flexible around working hours and be prepared to work some evenings or weekends as reasonably required by this role Be impartial and non-judgmental Have knowledge of the community support system in the Greater Boston area. Committed to the rights of seniors and disabled people to live dignified lives. Is flexible sensitive to the needs of others reliable and able to cope with emergencies. Prior home care experience helpful.
