If so, we have an exciting opportunity for you. We are looking for Housing Case Managers to join our team. In this role, you will provide vital community-based support, working with people in their homes and following a housing-first approach. We understand that individuals who have experienced chronic homelessness require additional support to achieve positive tenancies. As a full-time Housing Case Manager, you will play a crucial role in delivering ongoing case management support, utilizing your expertise in comprehensive assessment, planning, implementation, and overall evaluation of participant needs. Plus, your willingness to transport clients and their belongings in your personal vehicle, as well as your dedication to meeting with clients through community outreach, will make a significant difference in the lives of those we serve. While a Bachelor's Degree is preferred, we also welcome candidates with 2+ years of relevant experience in a related field. Join us in making a positive change in your community. As a Housing Case Manager, you will: Reach out to and assess individuals in the community as needed, in alignment with our mission. Evaluate and assess referrals, applying a housing-first framework and understanding the unique challenges faced by people experiencing homelessness. Collaborate closely with our shelter staff to ensure a seamless transition for guests moving from shelter to permanent supportive housing. Maintain accurate case records and collect data-based statistical information in a timely and organized manner. Create and implement individual service plans that prioritize participant goals within required timeframes. Monitor participant progress, conduct assessments to identify strengths, needs, barriers, and goals to promote housing sustainability and self-sufficiency. Provide essential education on daily living activities and refer participants to relevant service providers as needed. Complete home visits and assist participants in accessing community resources, scheduling and attending appointments (transportation may be required). Act swiftly to locate and secure housing opportunities for participants, often within tight timeframes. Build and maintain positive relationships with local landlords, property managers, housing authorities, social service organizations, and state agencies. Offer support, facilitate crisis intervention, mediation, and service coordination as necessary. Collaborate with internal and external partners to create a holistic support network. Participate in regularly scheduled supervision, agency, and community meetings and required training sessions.
