Jobs › Fairhaven, MA › Elder Outreach Case Worker Fairhaven Council on Aging Elder Outreach Case Worker Fairhaven, MA Apply Position Summary The Fairhaven Council on Aging is seeking a compassionate and resourceful Part-Time Elder Outreach Case Worker to provide direct assistance, case management, and advocacy to older adults and their caregivers. The position will help individuals access benefits, services, and community resources that support their safety, independence, and well-being. Responsibilities Assist older adults and caregivers with benefit applications, documentation, housing resources, and other assistance programs. Conduct home visits and provide outreach to older adults who may have difficulty accessing services. Assess client needs, make appropriate referrals, coordinate services, and provide follow up. Advocate for clients and assist them in navigating government and community service systems. Collaborate with Coastline Elderly Services, healthcare providers, housing agencies, legal services, and the Fairhaven Police Department?s Mobile Outreach Team. Maintain confidential client records and service documentation. Qualifications Experience in elder services, social services, case management, human services, or a related field preferred. Candidates should have strong communication, organizational, and problem solving skills and the ability to work compassionately with older adults experiencing complex needs. Knowledge of public benefits and local elder-service resources is preferred. A valid driver?s license and reliable transportation are required for home visits.
Position:
Approximately 12 hours per weekPay Rate:
