My Choice Programs is a well-established (Est. 1972) highly regarded, and growth oriented human service agency in Worcester, serving adults challenged with developmental delays, Acquired Brain Injuries (ABI) and highly functioning Autism Spectrum Disorders (ASD). My Choice Programs is driven by a philosophy of choice and committed to services which empower individuals to exercise that choice; make meaningful decisions; participate fully in their communities; and live independently. We are seeking to hire case managers to provide outreach services for individuals living in the community (
City of Worcester, and Southern Worcester County:
Milford, Webster. Franklin, Southbridge, Blackstone Valley Areas) either with their families, or independently. Our home office is located at 1158 Main St., Worcester, MA 01603. "If you don't want a job sitting in front of a computer screen all day, and would rather be in the community actively helping people, this job may be a great fit for you!" The position supports our served individuals in the community with mentoring in the areas of life skills/independent living skills training, medical management, advocacy, community access and integration. Use of your vehicle for local travel to appointments is required, with gas credit card provided . Basic computer proficiency and experience serving this population of individuals is favored, as is knowledge of the development and implementation of ISP's (Individual Support Plans) and Life Plans for individuals-served by the state DDS. Full-time benefits, including health insurance, 401K, vacation, personal, sick time off Flexible schedule . 37.50 hour work week, paid for 40 hours. Partial "Work from home" (hybrid) option for some responsibilities (most client contact is in-person). • Applicants need valid Mass. driver's license, good driving record, and reliable vehicle to visit/transport individuals served by the programs.

Good driving (not necessarily "perfect") record is a MUST for outreach positions.
