• Full-time About Us Boston Rehabilitation Center/Shore Point Recovery is a premier behavioral health facility dedicated to transforming lives through compassionate, individualized care.

Our Wareham facility is a 35-bed detox and residential treatment center, providing 24/7 support for individuals recovering from substance use disorders. Position Overview We are seeking dedicated and detail-oriented Case Managers to support clients in our 40-bed detox and residential program. Case Managers play a vital role in coordinating care, advocating for clients, and ensuring they have access to the resources needed for a successful recovery. This is a full-time, onsite position, SUNDAY-THURSDAY, 8am-4:30pm position.

MUST HAVE CASE MANAGEMENT EXPERIENCE IN SUBSTANCE ABUSE/DETOX TREATMENT.

Key Responsibilities

• Conduct comprehensive client assessments and develop individualized treatment plans.
• Coordinate discharge planning and aftercare services, ensuring continuity of care.
• Advocate for clients and assist them in accessing community resources, including housing, employment, and continued treatment.
• Collaborate with clinical and medical teams to support client's treatment goals.
• Facilitate case management sessions and provide ongoing support to clients throughout their recovery journey.
• Maintain accurate and timely documentation in compliance with regulatory standards.
• Monitor client progress and adjust care plans as needed.
• Provide education on relapse prevention, life skills, and available support services. Qualifications
• Experience in substance abuse treatment or behavioral health settings required.
• Bachelor's degree in social work, psychology, counseling, or a related field preferred.
• Knowledge of case management principles, discharge planning, and community resources.
• Strong communication, organizational, and problem-solving skills.
