Job Description

Join Our Team! Are you ready to transform lives and make a real difference in our community? We're looking for passionate individual to join us as Supported Community Living Case Manager. In this role, you'll be at the heart of empowering individuals living independently, providing vital support such as managing finances, coordinating medical appointments, and assisting with daily needs. Your impact will extend beyond tasks; you'll forge meaningful connections with individuals and collaborate closely with a supportive team of co-workers, clinical staff, and supervisors, all under the mentorship of our Manager of Shared and Community Living.
Compensation:
$20.75-$22.35 per hour (based on experience)
Location:
Yarmouth Port, MA Hours:
8:00am-3:00pm
Requirements:
High School Diploma or GED Bachelor's degree in human services or related field preferred Valid Driver's license The ability to physically intervene in instances of extreme behavior.
Key Responsibilities:
Assist and support individuals with their ADL skills, personal needs and community involvement. Oversee individuals' financial records and money management plan. Assist with meal planning, preparation and food shopping. Assist with planning and scheduling of healthcare, vocational and social activities. Complete monthly financial packets for submission to LCI business office according to established procedures and timelines. Provide case management supports to ensure a safe and healthy home environment. Facilitate transportation supports to ensure access to community resources, i.e. medical appointments, job interviews, etc. Provide non-violent crisis intervention and support as needed.
