Job Description

Case Manager
POSITION OVERVIEW
Responsible for assessing clients' needs and capabilities and assisting clients as they work towards their goals. Complete annual assessments, reassessments, and individualized service plans. Help ensure a smooth transition into their communities. Establish linkages to needed resources including, but not limited to medical care, vocational services, entitlements, legal assistance, housing, and family services.
ESSENTIAL FUNCTIONS
  • On behalf of clients, commit to being truthful and earnest, demonstrating fair and human respect in communication with family, significant others, and representatives.
  • Assist clients in receiving and maintaining entitlements.
  • Monitor progression toward client goal attainment.
  • Performs crisis intervention as needed.
  • Seeks housing placements for clients as needed.
  • Use creativity, judgement, and teamwork to address client concerns.
  • Provide referral, intake, and orientation processes with new clients.
  • Accompany clients to appointments as needed.
  • Always create positive and enjoyable interactions with clients.
  • Timely and accurate reporting of all documentation, treatment plans, contact notes, and any other documentation assigned.
EDUCATIONAL & JOB REQUIREMENTS
  • Minimum of an Associate's Degree in mental health or related field.
  • 1 year of work experience in mental health or related field
  • Excellent organizational skills and ability to prioritize tasks.
  • Provide flexibility to meet client needs.
  • Ability to succeed in a team environment.
