(CPR) is a private, nonprofit organization with a proven track record of providing behavioral health crisis services to individuals experiencing mental health and substance use disorders. Since 1992, CPR has delivered timely, person-centered crisis response and treatment services in the least restrictive environment. CPR serves Baltimore as the city's first and only comprehensive crisis response system of care, providing mobile crisis services, detoxification, case management, crisis residential services, and community-based supports. Program Overview CPR is committed to providing compassionate, comprehensive, and accessible 24/7 services for individuals experiencing substance use disorder (SUD) in Baltimore City. Through resource navigation, case management, and integrated support services, we help individuals move from crisis toward stability and recovery. We are seeking mission-driven, professional, and solutions-focused individuals who are passionate about reducing barriers to care and supporting vulnerable populations through trauma-informed and person-centered services. The Opportunity CPR is seeking a dedicated and compassionate Case Manager to support individuals experiencing substance use and behavioral health challenges through intensive case management and community-based support services. In this role, you will work directly with participants to develop individualized Person-Centered Service Plans (PCSPs), connect individuals to housing, healthcare, behavioral health treatment, and recovery resources, and provide ongoing support to help individuals transition from crisis to stability. This position operates within a multidisciplinary team that includes Peer Recovery Specialists, Clinicians, and community providers, and plays a vital role in improving outcomes for individuals across Baltimore City. What You'll Do Case Management & Client Support Conduct individualized needs assessments and develop Person-Centered Service Plans (PCSPs) with participants Maintain a caseload and provide trauma-informed, recovery-oriented case management services Assist individuals in accessing housing, healthcare, substance use treatment, employment resources, and community supports Support participants in navigating complex systems including social services, healthcare, legal systems, and housing programs Community Outreach & Engagement Provide services in community-based settings including shelters, outreach locations, and other non-traditional environments Participate in outreach shifts, including occasional evenings/weekends, as part of 24/7 program coverage rotation Provide transportation support using company vehicles when necessary to ensure access to appointments and resources Collaboration & Care Coordination Collaborate with Peer Recovery Specialists, clinicians, and external providers to ensure coordinated care and continuity of services Participate in case conferences, operational meetings, and multidisciplinary team discussions Documentation & Reporting Maintain accurate, timely documentation and data entry in accordance with program standards and funder requirements Track participant progress and update service plans as needs evolve Support program data collection and outcome tracking initiatives Qualifications Qualifications Education Bachelor's degree in Social Work, Psychology, or a related human services field required Experience Minimum of 2-3 years of experience working with individuals experiencing mental illness, addiction, and/or co-occurring disorders required Additional Requirements Valid driver's license required Must be willing to transport clients using company vehicles when necessary Skills & Competencies Strong interpersonal and communication skills Ability to provide trauma-informed and person-centered support Experience with case management systems and electronic documentation Strong organizational skills and ability to manage multiple priorities in fast-paced environments Commitment to ethical, recovery-oriented, and culturally responsive care Why Join CPR Medical, Dental, Vision Insurance
