Qualifications Licensed Clinical Social Worker Full Job Description Summary:
(LCSW required) The Grant Per Diem (GPD) Case Manager provides comprehensive case management services to veterans participating in the GPD program. This role involves assessing veteran needs, developing individualized service plans, connecting veterans with resources, and supporting them in achieving their housing stability, employment, and overall wellness goals. The Case Manager collaborates with VA staff, community partners, and service providers to ensure program success and veteran satisfaction.
Key Responsibilities:
Participant Assessment and Enrollment:
Conduct thorough assessments of veterans' needs, including housing, employment, healthcare, and psychosocial factors. Enroll eligible veterans in the GPD program and maintain accurate records of participant information. Develop individualized service plans based on assessment results and veteran goals.
Case Management:
Provide ongoing case management services to veterans, including housing support, employment assistance, and access to healthcare services. Monitor veteran progress, update service plans as needed, and provide regular follow-up to ensure veterans stay on track. Address barriers to stability by connecting veterans with supportive services such as mental health counseling, substance abuse treatment, and financial assistance. 3.
Resource Coordination:
Identify and connect veterans with community resources, VA services, and other support networks. Collaborate with VA staff, community partners, and service providers to develop and maintain a network of resources and opportunities for veterans. Facilitate access to job fairs, workshops, and other events that support veteran reintegration. 4.
Housing Support:
Assist veterans in securing and maintaining stable housing through the GPD program and other housing resources. Provide guidance on tenant rights and responsibilities, budgeting, and landlord relations. Support veterans in transitioning to permanent housing solutions. 5.
Data Management and Reporting:
Maintain accurate and up-to-date case files and participant records, ensuring confidentiality and compliance with VA policies. Track and report on veteran progress, outcomes, and program metrics. Prepare regular reports for management and stakeholders on program performance and veteran achievements. 6.
Collaboration and Communication:
Work closely with VA staff, community partners, and service providers to ensure a coordinated approach to veteran support. Participate in regular team meetings to discuss veteran progress, program challenges, and solutions. Communicate effectively with veterans, staff, and external partners to foster positive relationships and program success. 7.
Professional Development:
Stay informed about industry trends, best practices, and regulatory changes affecting veteran services and case management. Participate in training and professional development opportunities to enhance skills and knowledge. Contribute to the continuous improvement of the GPD program and services. Qualifications LCSW Relevant certifications are a plus. Proven experience in case management, preferably working with veterans or in a related field. Strong understanding of veteran issues, housing stability strategies, and supportive services. Excellent communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Proficiency in Microsoft Office Suite and case management software. Strong problem-solving and decision-making abilities
