Client Resource Officer New Hope Rehabilitation Center Windsor Mill, MD Job Details Part-time | Full-time | Contract From $22 an hour 18 hours ago Benefits Health insurance Professional development assistance Qualifications High school diploma or GED Organizational skills Human Services Social Work Associate's degree Full Job Description Job Summary The Client Resource Officer supports individuals enrolled in the Psychiatric Rehabilitation Program by helping them access housing, public benefits, healthcare coverage, identification documents, and other community-based resources that promote stability, recovery, and independence. This role is responsible for connecting clients with benefits, transportation, identification renewal, healthcare coverage, and other support services while maintaining accurate documentation and following program guidelines. This position serves as a link between clients, the program, and external service providers to reduce barriers to care and ensure clients receive timely support. The Client Resource Officer also maintains accurate client documentation and assists with recordkeeping in accordance with program standards and applicable PRP requirements. Duties and Responsibilities Assist clients with locating and applying for safe and appropriate housing resources. Help clients access community resources such as food assistance, transportation, healthcare, employment support, and financial assistance programs. Support clients with applications, renewals, and follow-up for Medicaid, Social Security benefits, identification cards, and other required documents. Monitor client needs and connect them with appropriate agencies, providers, and support organizations. Maintain accurate, complete, and timely documentation of client contacts, referrals, applications, and outcomes in client records. Review client files to ensure required forms, signatures, consents, and supporting documents are current and complete. Educate clients on available resources and encourage self-advocacy, independence, and active participation in service planning. Follow up with clients regarding expired or missing documents, benefits, or other items needed for service continuity. Communicate with rehabilitation staff, and outside agencies to coordinate services and support client progress. Participate in team meetings, case reviews, and other program activities as assigned. Assist the program with maintaining compliance with internal policies and applicable state regulations related to client documentation and service coordination. Qualifications and Experience High School Diploma or GED required; Associate or Bachelor's degree in Human Services, Psychology, Social Work, or a related field preferred. Experience working with adults in behavioral health, psychiatric rehabilitation, social services, case management, or community outreach preferred. Knowledge of housing resources, public assistance programs, Medicaid, and community-based support services strongly preferred. Ability to build rapport with clients from diverse backgrounds and work effectively with community partners. Strong communication and interpersonal skills. Proficiency in Microsoft Office and client data management systems. Ability to manage multiple tasks and prioritize in a fast-paced environment. Knowledge of community resources, behavioral health, and addiction recovery is an asset Ability to maintain accurate records and handle confidential information appropriately. Strong organizational skills and attention to detail.
