Case Manager The Connection Youth Services Program (TCYS) Full-time, non-exempt About Livingston Family Center, Inc. Livingston Family Center (LFC) is a nonprofit agency focused on providing quality community-based services that include outpatient mental health treatment, runaway and homeless youth emergency shelter and crisis intervention, and supervised visitation and exchanges for families affected by domestic violence. Position Responsibilities The Connection Youth Services' Case Manager has the responsibility to provide intensive, coordinated case management services to all runaway, homeless, and at-risk youth and their families who enter and exit The Connection Youth Services' Street Outreach and Transitional Living Programs. The Case Manager facilitates a wraparound approach to case planning with youth, their natural support system, and community-based services such as community mental health, school administration, and other human service agencies. The Case Manager is an essential part of TCYS' Street Outreach team; working to ensure youth and families are connected with resources necessary to maintain a safe, stable environment. General Case Manager Responsibilities Knowledge and compliance with all LFC agency and TCYS program policies and procedures and privacy practices Commitment to Diversity, Equity, and Inclusion and promotion of an environment that is welcoming to all Participate in the 24-hour line rotation to provide crisis intervention, referral information, and screening of potential TCYS clients Engage with youth and families utilizing evidence-based frameworks including, but not limited to, positive youth development, trauma-informed care, and strengths-based perspective Provide intensive, coordinated case management and support services to youth and families Facilitate and document plan and goals meetings with youth, families, and natural support system Provide aftercare and community-based case management services to youth and families Assist youth, families, and support systems to develop and implement treatment and discharge plans Complete face-to-face intake with each client within the required guidelines Assist youth to obtain vital documents, apply for healthcare benefits, and enroll in education and/or vocational services based on youth's service plan Facilitate weekly youth support groups as assigned, including Life Skills group developement Participation in staff and supervision meetings Assist with Drop-In Center staff coverage Completion of training requirements Participation in community collaboration and workgroups as appropriate Participation in agency outreach and fundraiser activities Maintain general cleanliness, organization, and safety of facilities Additional responsibilities may be assigned as necessary by the Program Director, Executive Director, and/or Board of Directors Outreach Responsibilities Assist in dispersing agency information flyers, event announcements, and promotional items through social media, public events, and community presentations Cultivate positive community relationships with volunteers, interns, and donors Assist with recruiting, vetting, and supervising volunteers, unpaid interns, and youth seeking community service opportunities Assist with organizing and dispersing donations including holiday gifts and meals Data Responsibilities Maintain data entry systems (HMIS, electronic health records) on all assigned clients Knowledge and compliance with all MDHHS, FYSB, and other funders' reporting requirements Maintain a chart for each client as required within time requirement guidelines Utilize assessment tools such as the Casey Life Skills assessment to develop treatment plans and monitor client progress Develop and maintain community-based resource file of various agencies and services that assist youth and families Assist with creating monthly, quarterly, and annual program progress and data reports Assist with completing and compiling 90-, 180-, and 365-day follow-up interviews Compile youth pre and post survey responses; share program improvement suggestions based on data compilation Compile any additional data to monitor and assess agency and runaway/homeless youth trends Qualifications The Case Manager will be committed to promoting an inclusive work environment and positive team contribution.
The preferred candidate will have:
Bachelor's degree in the human services field Two years of experience working with young people and families Valid driver's license and car insurance Knowledge and experience with social media platforms, Microsoft Word, Excel, Publisher, and PowerPoint software Successful completion of all required criminal checks and health screenings CPR/First Aid Certification (agency may provide reimbursement for completion) Must be willing to work evenings and weekends as needed Benefits and Compensation The Case Manager is a full-time, non-exempt position. The starting salary is $45,760 a year and will be commensurate with education, qualifications, and experience.
