The Case Manager works with individuals, families, and providers to coordinate services and ensure individuals receive the supports needed to live safely and independently in the community. SSIS Qualifications Required:

Bachelor's degree in Social Work, Psychology, Human Services, or related field Valid driver's license

Preferred:
