Monday - Friday, 8am to 4pm (occasional evenings and weekends) Reports to: Prevention and Care Program Manager For 39 years, the Indigenous Peoples Task Force has been at the forefront of closing the health equity gap for Native American and other marginalized communities impacted by
HIV/AIDS
across the Twin Cities and greater Minnesota. Based on Indigenous values and ways of knowing, our programs have grown to include services that promote integral role of and return to the centrality of first medicines, the use of cultural arts to support youth development, and programs that foster food security in ways that reconnect Indigenous and other peoples to the land and waters. Job Summary The Housing Case Manager I provides extensive case management services using evidence-based, trauma informed approaches to unsheltered and insecurely housed gain access to emergency shelter. Collaborates with internal and external partners, to seamlessly, guide people to affordable and secure housing. Partners with adults and adults with children to maintain their current housing through vouchers and other supportive services connections. Minimum Qualifications Demonstrated experience managing a high need caseload. High school diploma with experience working with people at the intersection of homelessness and comorbidities, including but not limited to opioid and chemical dependency challenges. Understand the impact of homelessness on Indigenous communities, in urban and rural Minnesota. A working knowledge of housing systems and resources. Four or more years of experience in any or a combination of the following areas: case management, housing, knowledge of housing systems, public health engagement and promotion, outreach in Indigenous and other communities of color, the LGBT+ communities. Experience successfully collaborating and communicating with diverse stakeholders; and advocating for clients across multiple systems. Preferred Qualifications Bachelor's degree in human services, social work, sociology, psychology, anthropology, any other closely related field; or a bachelor's degree. Demonstrable knowledge of and experience working with Indigenous communities Knowledge of the Minnesota housing challenges and resources Working knowledge of and experience with navigating the housing systems Experience navigating database software and platforms including but not limited to HMIS and Coordinate Entry Experience providing services through a trauma informed care and harm reduction framework Possess a valid Minnesota driver license Ability at problem solving Ability to work independently and collaboratively Experience with social media networking and outreach Excellent verbal and written communication skills Proficient with Microsoft Office Suit and database management systems Commensurate with experience and qualifications.
