Non‑Exempt Job Summary SEMCAC is seeking a dedicated and compassionate Outreach Services Case Manager to provide vital services to individuals and families in our communities. This position supports households by assessing needs, determining eligibility, completing applications, providing referrals, and offering ongoing case management to help clients achieve stability, independence, and self‑sufficiency. Essential Duties & Responsibilities Provide intake, application assistance, case management, referrals, and follow‑up services Maintain knowledge of Semcac and community resources to best serve clients Advocate for low‑income individuals and households facing economic barriers Manage multiple client caseloads while following program guidelines and funder requirements Build and maintain collaborative relationships with service providers and community partners Assist with housing‑related services including homelessness prevention, transitional housing, motel vouchers, and eviction prevention Conduct data entry and case documentation in HMIS, THO, and other required systems Participate in outreach activities and community education efforts Supervise volunteers or staff as assigned Attend required trainings, workshops, and meetings Qualifications Two‑year college degree in Human Services or related field preferred, or equivalent education and experience 2-4 years of related experience preferred Strong communication, organization, and advocacy skills Ability to manage competing priorities and work effectively in a fast‑paced environment Proficiency in Microsoft Office 365 and basic computer systems Experience working with diverse populations and community partners Valid driver's license or ability to travel for home visits, meetings, and trainings Skills & Abilities Client‑centered and nonjudgmental approach Strong problem‑solving and decision‑making skills Ability to handle sensitive situations with professionalism and empathy Capable of working both independently and as part of a team Why Work at Semcac? Pay:
$21.08 - $23.45 per hour
