Job Description

Job Overview PDS is seeking a compassionate, organized, and motivated Case Manager to support individuals and families in accessing essential services and achieving greater stability. The Case Manager will assist clients with applying for SSI/SSDI, county benefits, health insurance, housing, employment, and other community resources. This position works closely with clients, community partners, probation officers, healthcare providers, and social service agencies to coordinate services, remove barriers, and help clients meet their goals. The ideal candidate is skilled in client advocacy, resource coordination, and case planning while maintaining a client-centered and strengths-based approach. Key Responsibilities Conduct client intakes and assessments to identify needs and goals.
Assist clients with applications for:
SSI/SSDI
benefits SNAP (Food Support) Medical Assistance and other health insurance programs General Assistance and county benefits Help clients access housing resources, housing applications, and housing placements. Support clients with employment searches, job readiness, and connections to workforce resources. Develop and monitor individualized service plans based on client needs and goals. Coordinate services with internal staff, county agencies, probation officers, healthcare providers, and community organizations. Make referrals to mental health, chemical health, healthcare, employment, housing, and other support services. Maintain accurate and timely case notes, client records, and program documentation. Monitor client progress and provide ongoing support, advocacy, and follow-up. Participate in team meetings and collaborate with colleagues to ensure quality client services. Maintain confidentiality and comply with agency policies and privacy regulations. Qualifications Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field preferred. Experience working with individuals experiencing homelessness, poverty, mental health challenges, substance use disorders, or justice system involvement preferred. Knowledge of SSI/SSDI, county benefits, housing resources, employment services, and public assistance programs. Strong communication, organization, and problem-solving skills. Ability to work independently and as part of a team. Proficiency with computers, Microsoft Office, and electronic case management systems. Valid driver's license and reliable transportation preferred. Pay:
$22.37 - $24.00 per hour
