Summary:
The Waiver Case Manager provides comprehensive case management services for individuals that are aging and/or have been diagnosed with a disability. Typical case load includes individuals with complex medical diagnoses, mental health diagnoses, developmental disabilities, and those who are aging. This position requires meeting Disability Service program requirements as outlined by Ramsey County Human Services, Hennepin County Human Services, and Jewish Family and Children's Service.Agency Information:

Approximate Annual Salary $50,700-54,600 Competitive benefits package including medical insurance, 401(k) with match, life and long-term disability insurance, and access to other benefits including vision and dental Work-life balance including 37.5 hour work week, 3 weeks of vacation, paid sick time, paid parental leave, holidays, flexible schedule, and the option to work up to 60% time from home Social work licensing supervision and license renewal feesResponsibilities:
Case management for assigned clients onCADI, DD, CAC, BI, EW
waivers, Alternative Care (AC) program, or with Rule-185 or VADDTCM eligibility. Model and foster behavior that establishes a culture of diversity, equity and inclusion. Interpret and review assessment results and information from MNChoices assessment. Using person-centered approach, create unique care plan based on assessed needs. Identify services and support needed as well as goals and dreams of individuals. Update care plan as needed. Coordinate and manage service and support as outlined in the care plan. Make referrals to other agencies (formal and informal) as appropriate. Monitor waiver budget and service agreements to ensure payment of contract providers. Assist with transitions including hospitalizations, rehabilitation, and nursing home placements. Ensure that all necessary documentation is completed and signed as required. Complete chart audit corrections to ensure compliance with county and DHS requirements. Complete timely and accurate documentation, maintain complete and professional case records. Attend county disability service training and meetings as required. Participate in team meetings and case consultation discussions. Participate in committees (JFS, community partnerships) as assigned. Attend agency staff meetings, staff development activities and special events. Provide backup coverage for the Disability Services program and other Senior and Disability Service programs as needed. Maintain professional and ethical competency by adhering to agency personnel practices, HIPAA rules, the NASW Code of Ethics (if applicable) and other designated practices and procedures. Complete necessary tasks in a prompt and efficient manner. Other duties as assigned. Minimum Qualifications :

compassion, inclusion, innovation, integrity, and collaboration. Bachelor's or Master's Degree in Social Sciences, Human Service, Social Work, Nursing or other related field. Applicable licensure if required by degree. Supervised work in case management and experience with disability service programs is preferred. Experience working with clients with complex mental and physical health conditions necessary. Knowledge of county programs and community-based services preferred. Ability to qualify as a QDDP - Qualified Developmental Disability Professional - preferred. Intermediate computer skills, including typing and Microsoft Office experience. Able to work well in a multi-disciplinary team. Valid driver's license and a personal vehicle to use for agency business. Must live in commutable distance to JFS. Must pass background check. Actively contributes to the agency community by participating in committees, workgroups, or other collaborative efforts that support agency goals and foster a positive work environment. Work Environment/Physical Demands Regularly sits at desk or conference table and uses computer keyboard. Ability to bend, stoop, and reach to obtain files, equipment, etc. Frequent oral and written communication with staff and public, both one-to-one and in group meetings. Awareness of and ability to manage time to meet deadlines and complete work. Frequent interruptions and need to handle multiple tasks simultaneously. Indoor office setting. Frequent formal and informal interaction with others. Frequent use of telephone, computer, fax and copier, flip charts, and other tools for presentations. Must be able to walk up and down stairs, ability to lift up to 25 pounds, and get in and out of a car. Occasional exposure to inclement weather. Constant work with and around others. Must have use of personal transportation for occasional driving, current driver's license, auto insurance with appropriate coverage limits and clean driving record.
