EXAMPLES OF WORK
Ensure complete assessments are conducted on Veteran's social, environmental, and economic backgrounds to assist medical and other professional staff with diagnosis, prognosis, and treatment planning Ensure medically-related social services are provided for Veterans to attain or maintain the highest practicable physical, mental, and psychosocial well-being. Perform duties of Director of Social Services when absent from facility to include supervision and direction of Social Services team Provide medically related social services for Veterans to attain highest physical, mental and psychosocial well-being through the assessment and care planning process Identify and seek ways to support Veterans' individual needs and preferences, customary routines, concerns and choices Make arranges for obtaining adaptive equipment, clothing, and personal items Maintain contact with families to report on overall health and planning Make referrals and obtain services from outside entities Assist with and make referrals for financial and legal matters Provide appropriate level of discharge planning Provide or arrange for the provision of needed counseling services, participating in behavior management and crisis intervention and planning Introduce Veterans and families to the Home, providing information and giving tours Qualifications- All you need for success
QUALIFICATIONS
Licensed Senior Clinical Caseworker- Possession of a current license to practice as a Licensed Social Worker in the state of Missouri Licensed Bachelor Social Worker
