Full-Time Case Manager / Social Worker Jewish Family Services of Western North Carolina (JFS) Asheville, NC | Full-Time | $54,000-$57,000 annually Make a Difference in the Lives of Individuals, Families, Older Adults, and Caregivers Jewish Family Services of Western North Carolina (JFS) is seeking a compassionate, organized, and experienced Full-Time Case Manager to join our growing team. This position is ideal for a social worker who enjoys direct client service, connecting people with resources, collaborating with community partners, and working with older adults and caregivers. The Case Manager provides comprehensive case management to individuals and families across the lifespan and oversees Faye's Place Elder Club , JFS's group respite and social engagement program for older adults, including individuals with mild to moderate cognitive impairment. What You'll Do:
Case Management Assess client needs, strengths, available supports, and eligibility for services. Develop individualized service plans and help clients work toward identified goals. Connect clients with resources for housing, food, transportation, healthcare, mental health services, benefits, financial assistance, caregiver support, and other needs. Provide ongoing case management through office, telephone, virtual, and home-based contacts. Advocate for clients and assist them in navigating community services and benefit programs. Collaborate with JFS counseling, senior services, and other programs to provide coordinated support. Maintain accurate and timely case notes, service plans, referrals, and required documentation. Develop strong working relationships with community organizations, referral partners, and service providers. Faye's Place Elder Club Oversee the day-to-day operation of Faye's Place Elder Club programming in Buncombe and Henderson counties. Assess prospective participants and work closely with their families and caregivers. Monitor participant well-being, changing needs, and continued program appropriateness. Plan and coordinate engaging social, recreational, educational, cultural, music, arts, and exercise activities. Coordinate participant schedules, attendance, meals, transportation, supplies, and program activities. Recruit, orient, schedule, and support Elder Club volunteers. Connect participants and caregivers with additional JFS and community services when needs are identified. Help maintain a safe, welcoming, engaging, and person-centered environment for older adults.
What We're Looking For Required:
Bachelor's or Master's degree in Social Work ( BSW or MSW ). Social work or case management experience in a social service agency or similar setting. Strong interpersonal, communication, organizational, and computer skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Valid driver's license, reliable transportation, and insured automobile. Ability to travel locally between JFS program locations and client homes.
Preferred:
Master's degree in Social Work. Experience working with older adults and caregivers. Knowledge or experience related to dementia, Alzheimer's disease, or cognitive impairment. Experience planning, facilitating, or leading group activities. Familiarity with community resources and programs such as Social Security, Medicaid, SNAP, and county social services.
Compensation & Benefits Salary:
