Full Job Description Description:
As an Acute Case Manager, you will be responsible for working with patients to ensure they receive the best possible care and treatment. You will be working closely with doctors, nurses, and other medical professionals to coordinate patient care and ensure a successful outcome.
Your duties will include:
Assessing patient needs and developing individualized care plans Coordinating with medical professionals to ensure proper treatment and care Advocating for patients and their families to ensure their needs are met Monitoring patient progress and adjusting care plans as necessary Providing education and resources to patients and their families Maintaining accurate and up-to-date patient records Other duties as assigned. Requirements:
A Bachelor's or Master's degree in Nursing or a related field Active nursing license At least 2 years of experience in case management or a related field Excellent communication and interpersonal skills Strong organizational and time management skills Ability to work independently and as part of a team Knowledge of medical terminology and procedures
