This position will require collaborative work with other team's members, and services will be primarily delivered in the consumers place of residence or in the community. This position requires exceptional assessment, verbal, interpersonal, and writing skills. Must pose strong time management skills. Applicant must have a minimum of 1 year experience with the adult MH/SA population. Team of 5 staff up to 48 clients Document supervision to meet state and agency standards; Insure that the
Team:
consults with identified professionals, family members and others, includes their input into the Person Centered Planning process, informs all involved stakeholders, and monitors the status of the recipient in relationship to the treatment goals; Conduct a full CS Team Meeting with each assigned CST consumer monthly; Meet with initial CST consumers weekly for the first 30 days (may be less for consumers already attending CST); Conduct individual Case Reviews with QPs quarterly, or more often as needed, in order to assist with development of PCPs and monitor services; Provide ongoing support to team Monitor staff compliance with Medicaid standards and protocols; Monitor authorization dates for your team Complete updates and Addendums for all authorizations for the team Lead team on treatment modality with interventions Conference with dissatisfied and concerned consumers regarding their services; Weekly Supervision with Program Manager and CST team members
