The Bridge Behavioral Health is a nonprofit behavioral health organization providing medically monitored withdrawal services, residential treatment, outpatient care, and mental health respite services. Our trauma-informed team supports clients through every stage of recovery — regardless of their ability to pay. We believe recovery is possible for everyone, and our staff are the heart of that mission. What You'll Do As a Case Manager, you'll help clients navigate treatment, access resources, and build a path toward long-term recovery. You'll work closely with a multidisciplinary team while creating meaningful relationships with clients and families. This role also supports the admissions and referral process by helping coordinate assessments, authorizations, and transitions into appropriate levels of care.
In This Role, You Will:
Support clients in a welcoming, safe, and trauma-informed environment Build professional, supportive relationships with clients Coordinate care with probation, parole, therapists, family members, and community providers Assist clients with referrals, housing, appointments, and community resources Lead psychoeducational groups in residential treatment Support clients through admissions, treatment planning, continued stay reviews, and discharge planning Complete intake assessments, biopsychosocial evaluations, and screening tools as appropriate to credentialing level Coordinate continued stay reviews, utilization reviews, and insurance authorizations to support medical necessity and continuity of care Serve as a point of contact for referral sources and assist with communication regarding admissions and placement coordination Conduct assessments within community settings, including jails, hospitals, and partner agencies, to help facilitate direct transitions into treatment Support admissions scheduling, bed coordination, and program placement to improve access and operational flow Assist in tracking program availability and helping determine appropriate placement based on clinical presentation and level of care needs Support continuity of care and transitions from residential treatment into outpatient services, including Intensive Outpatient Programming (IOP) Collaborate with multidisciplinary staff to improve admission efficiency, discharge planning, and client engagement Help families navigate behavioral health services and recovery supports Provide crisis support and ongoing client assessment Complete timely and insurance-compliant documentation in the electronic health record (EHR) Participate in clinical staffing meetings, trainings, and treatment team collaboration Assist with transportation when needed What We're Looking For Minimum Qualifications Associate degree required (Human Services or related field preferred) Must be at least 21 years old Eligible to work in the
