Job Description

Housing Support Tenant Liaison- The Housing Supports Housing Liaison helps individuals and families experiencing homelessness or at risk of homelessness to secure stable housing quickly in Atlantic & Cape May Counties. This role involves conducting assessments, developing service plans, connecting clients with housing opportunities, and coordinating resources to promote long-term stability. Services include pre-move support, housing stabilization, home safety, and relocation assistance. This position provides site-based and community outreach services while collaborating with landlords, developers, and community partners to expand housing resources.
Key Responsibilities:
Maintain a weekly average of face-to-face client contacts as required by program standards. Complete all client documentation by the end of the shift, ensuring timeliness, accuracy, and compliance with program requirements. Maintain accurate and up-to-date client records in the agency's Electronic Health Record (EHR) system. Ensure accurate documentation of intakes, service plans, progress notes, and applications in agency systems. Conduct intakes and create action plans to move clients into permanent housing efficiently, reducing the time they spend in crisis. Assist clients in finding appropriate housing matches to promote stability and support their long-term goals. Provide site-based and community outreach case management services to individuals and families at risk of or experiencing homelessness. Assist clients in securing and maintaining stable housing, including pre-move assistance, home safety planning, and relocation support. Provide ongoing case management, including regular follow-ups and service plan adjustments.
Position Requirements:
Bachelor's degree in Social Work, Human Services, or a related field preferred; Associates degree with relevant experience accepted. Experience in housing support, case management, or working with individuals experiencing homelessness preferred Familiarity with Medicaid documentation and community housing programs Valid NJ Driver's License required At JFS, you'll be part of a passionate, diverse team that's been making a real difference in the community for nearly 100 years.
