EXPERIENCE/KNOWLEDGE/SPECIALIZED TRAINING
Must have a BA or BSW with prior case management experience. Residential experience, experience in domestic violence preferred. Bi-lingual Spanish skills preferred. Knowledge and sensitivity to issues of domestic violence, substance abuse and homelessness and a commitment to women's empowerment and the eradication of racism. Excellent verbal, written, and public speaking skills. Knowledge of computer programs including but not limited to Microsoft Office Suite (Word, Excel, Power Point, Outlook). Valid Driver's License and reliable transportation. Maintain flexibility in schedule to meet client needs. Be available for some evenings and weekends.
ESSENTIAL JOB DUTIES
Responsible for ensuring the following services are provided to clients: case management, advocacy, and referral. Responsible for overall management and coordination of Supportive Housing program. Provides Director of Residential Services with information on potential problems and assists in the development of solutions and/or innovative approaches to problems. Exhibits sensitivity and concern when interacting with residents. Responsible for helping to create an empowering and safe living environment for clients. Provides and maintains accurate client records and reports. Implements the Supportive Housing program in collaboration with the Director of Residential Services.
Provides the following direct services:
intake/exit interviews, weekly meetings, referrals, advocacy, and development of case management plans for each client. Maintains contact with community agencies to coordinate the delivery of services. Refers clients to appropriate community resources for services not covered by YWCA Union County. Accompany clients to community resource appointments as needed. Responsible for following up with clients who have missed appointments and documenting contacts in the case record. Acts as liaison with apartment site managers. Completes necessary reporting requirements for all program areas by assisting in reporting statistics and in preparation of reports. Acts as liaison to the YWCA collaborating partners in providing the following services to clients: career counseling, housing, and job development, and others. Assists in client application process including the review and acceptance of clients into the SH program as requested. Documents and maintain case records for individual clients, completes service schedules, and maintains other records as needed. Assists clients in move-in process to apartments as needed, as well as assist in securing necessary household items/furniture beyond that which is supplied by YWCA. Conducts regular house checks. Works to maintain a neat, clean, and safe environment and assist clients in doing so. Maintains a system of accountability to ensure proper statistical information gathering and analysis. Notifies Director of Residential Services when unusual cases, irregularities, accidents, or illnesses occur. Assist clients in accessing services as required. Maintains and updates referrals and relationships with agency's contacts. Attend case management meetings, staff meetings, trainings and conferences as directed. Attend community meetings as required. Other duties and responsibilities as needed and directed by the Director of Residential Services. All positions within the shelter will be trained on answering the hotline and for shelter coverage. Occasional second or third shift work may be required. Responsible for participating in agency meeting requirements, especially where such meetings intersect with the needs of the PALS program and the needs of children surviving domestic violence. Responsible for building/maintaining a bridge between PALS and the other programs in the agency.
