Job Description

Are you interested in helping adults with intellectual and developmental disabilities in the community? Community Care Partners is looking for a compassionate and organized Case Manager to oversee an assigned caseload of individuals receiving Community Based Supports, Individual Supports, and Supported Employment services through the Division of Developmental Disabilities (DDD). As a Case Manager, you will coordinate services, support individuals in achieving their goals, and work closely with individuals, families, Direct Support Professionals (DSPs), and Support Coordination agencies to ensure quality, person-centered services. Responsibilities Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Handle case assignments, draft plans of care, and attend ISP meetings. Complete training by RN on medical protocols; to then train DSPs. Supervise assigned Direct Support Professionals (DSPs) in the field. Help individuals feel empowered and involved in the decision-making process. Develop effective working relationships with Support Coordination agencies. Maintain regular contact with individuals on your assigned caseload. Conduct visits with clients and families as needed. Review activity logs submitted by Direct Support Professionals to ensure accuracy and compliance. Adhere to agency policies and procedures. Attend relevant professional development trainings as needed. The employee will be responsible for conducting visits at clients' homes and other service locations. Weekly mileage reimbursement is provided. Skills & Qualifications Proven experience working with individuals with intellectual and developmental disabilities (I/DD). Knowledge of Supports and Community Care Program manuals preferred. Previous supervisory experience preferred. Excellent communication and interpersonal skills. Strong organizational and time management skills. Problem-solving skills and ability to multitask. Current valid driver's license and reliable transportation. Bachelor's degree in a related healthcare or human services field preferred.
