COPY LINK link Adecco is assisting a local client in Hamilton, NJ in their search for a Case Manager. This is a part-time role (32 hours a week) paying $24/Hour. If qualified, below!ESSENTIAL JOB RESPONSIBILITIES
- Responsible for connecting with individuals and families in need of services and support.
- Determines individuals' household ie for eligibility for emergency assistance with back rent, utility, and food assistance; based on eligibility, facilitates client enrollment into grant-funded programs.
- Develops and facilitates linkages to other service providers; builds a network of resources to support clients' needs beyond our scope.
- Manages relationships with other local nonprofits; participates inmunity outreach initiatives.
- Communicates with clients regularly, via phone or in-person; maintainsputer-based service plans to monitor client progress.
- Manages client files and inputs daily/weekly case notes; prepares monthly/quarter reports and participates in quarterly audits.
- Participates in the development of client-centered programs and services; monitors and promotes client participation.
- Interacts and participates as a member of the team and performs otheranizational duties as assigned.
REQUIREMENTS
- Must be punctual, reliable, and able to work independently, with minimal supervision.
- Must have exceptional written, verbal, and problem-solving skills. Case Manager Job Description
- Must be able to work with diverse populations; being sensitive to cultural, economic and gender differences.
- Must be able to manage time and respond to deadlines as assigned.
- Must know how to operate basic office machinery/equipment (scanner, copier, fax, er).
- Knowledge of Mercer County social service system(s), with a network of resources; a plus.
- Bi-lingual (Spanish); a plus.
QUALIFICATIONS
- Bachelor's degree with major course work in social work, sociology, psychology or counseling.
- Minimum three (3) years of demonstrated casework/client management experience, providing support services to targeted populations.
- Knowledge of grants and grant management.
- Proficient in Microsoft Office (Outlook, Word, Excel, and Power Point), Adobe Acrobat, and social media platforms.
- Must possess a valid NJ Driver's License; occasional driving required.
- Must be able to lift up to 30 lbs.
Pay Details:
