JOB SUMMARY

— Case Manager, HEDS Program (Atlantic County, NJ) We're seeking a compassionate and dedicated Case Manager for the Home Electronic Detention System (HEDS) program. In this role, you will support youth involved with the juvenile justice system and their families.

What You'll Do:

Meet with youth and their families multiple times a week to offer support, education, and linkages to resources Work with youth and guardians to develop individualized plans addressing behavior, emotional health, relationships, and legal challenges Conduct intakes and risk assessments for new referrals Manage a small caseload (up to ~6 high-risk youth on HEDS) Provide case management and referrals to community-based services Lead family empowerment groups Be on call to support youth and families as needed Maintain thorough documentation, including individualized goal plans Collaborate with courts and probation, including providing reports and advocacy Participate in community outreach and our agency's cultural diversity efforts Position Requirements Education/Experience- A Bachelor's degree in Social Work or a related field. Two years of experience in the mental health field preferred. Flexibility to work evenings. Bilingual English/Spanish a plus.
