Community Health Worker Piscataway, NJ, United States | req7238 Posted on: 6/1/2026 Back to Search Community Health Worker QIP-NJ The Community Health Worker will: Assist in coordinating patient care. Collaborate with members of delivery team (Doctors, Social Workers, Nurses) to identify concerns that would prevent quality medical care. Conduct outreach activities in targeted areas. Establish positive, supportive relationships with patients/clients in managing their health care needs. Provide comprehensive case management. Provide effective and productive communication between patients, providers and supervisor.
Requirements:
Health/Social Services experience or education Excellent verbal and written communication skills with focus on customer service Valid NJ driver's license Case management experience preferred Strong computer skills Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population Ability to work both independently and as part of a team; Strong organizational, case management and interpersonal skills Bilingual candidates a plus
Salary Range:
18.00 - 26.58 USD We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
