Job Description

Are you interested in helping adults with intellectual disabilities in the community? Community Care Partners is looking for TWO (2) Case Managers fluent in English and Spanish to oversee assigned caseload of individuals receiving Community Based supports, Individuals supports and Supported Employment from the Division of Developmental Disabilities (DDD). Responsibilities Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered Handle case assignments, draft plan of care, review activity logs, attend ISP meetings Supervise assigned Direct Support Professionals (DSP) in the field Help individuals feel empowered in the decision-making process Develop effective working relationship with Support Coordination agencies Provide a minimum of monthly contact to individuals on assigned caseload Maintain monthly contact with individuals on your caseload and conduct quarterly visits with clients and families Review activity logs submitted by Direct Support Professionals to ensure accuracy Adhere to agency policies and procedures Attend relevant professional development trainings as needed The employee will be responsible for conducting visits at the client's home. We offer a weekly mileage reimbursement. Skills Proven working experience work with individuals with intellectual and developmental disabilities (I/DD) Knowledge of Supports and Community Care Program manuals Previous supervisory experience Effective communication skills Excellent organizational and time management skills Problem-solving skills and ability to multi-task Current driver's license Bachelor's degree in related healthcare field preferred Join our team and make a real impact in the lives of individuals and families across New Jersey!
